FOR ACTIVE DCP MEMBERS -

REGISTRATION PROCESS:

1. Download, complete and save the editable 22/23 REGISTRATION FORM (PDF) provided below.

2. Submit your saved 22/23 REGISTRATION FORM in PDF format via email to: communications@dcpkids.com

3. Once sessions have been confirmed via email, process your registration fee for the 22/23 school year by clicking on the REGISTRATION FEE link provided below. Payment should only be made upon confirmation of enrollment; please wait to pay if you have been waitlisted.

INSTRUCTIONS FOR SUBMITTING REQUIRED FORMS

22/23 SCHOOL YEAR

As an active DCP member you only need to complete the following forms ONCE unless otherwise noted:

ODJFS Child Medical Statement (must be submitted each school year)
ODJFS Child Enrollment and Health Information (unless changes are necessary, you will only need to sign off on the copy already on file. Date for document review TBD)
ODJFS Request for Administration of Medication (if applicable)
ODJFS Child Medical-Physical Care Plan (if applicable)

All forms can be submitted in your Jovial Family Portal.
